Appointment Policy
Call our office at (606) 373-0316 to schedule an appointment. Online scheduling is also available through the patient portal. Please arrive a few minutes early for your visit so we can check you in and get started on time.
Cancellation Policy
If you need to cancel or reschedule please call us as soon as possible. We ask for at least 24 hours of notice when you can so that another patient can take that appointment time.
Late Arrival Policy
If you arrive more than 15 minutes late for your appointment we may need to reschedule your visit. Please call us on the way if you are running behind and we will do our best to work with you.
Prescription Refill Policy
Prescription refills for established patients can be requested by calling our office during business hours. Please call before your prescription runs out. Controlled substances require an in-person visit.
Payment Policy
Co-payments and any balances are due at the time of your visit. We accept most major insurance plans. If you have questions about your bill please call our office.
After Hours and Emergencies
If you are experiencing a medical emergency call 911 or go to your nearest emergency room immediately. For urgent after-hours questions please call our main number at (606) 373-0316.
